Abstract

Industry competition in a variety of finished goods in the market is a product of the cooperation are complementary and mutually prioritizing among industries of various countries. Defects in a product is aproblem that must be considered by the company. Failure on the collar pipe products in PT. Bannex Indonesia 3.930 pcs of total production of 15.650. That results are higher than company’s tolerance which is 5% of the production. The research uses a method failure mode and effect analysis (FMEA). FMEA is a structured procedure to identify and prevent as much as possible model of failure (failure mode). The types of product failure pipe collar; bending, burry, and out of dimensions. Based on the results of the brainstorming that has been done has gained root of the problems that occur in the product the caused the collar pipe cutting machine that is the absence of regular preventive maintenance schedule that could cause burry. Cost defect inflicted on burry the highest defect in the process cutting.
