Abstract
Management of PT. Indotech Mitra Presisi still has problems, namely the high frequency of work accidents occurring in 2019-2021, amounting to 17, 19, and 16 per year. The occurrence of work accidents makes the loss of working hours foremployees and affects the decline in productivity. This study uses the Fault Tree Analysis (FTA) method to analyze the aims of finding the frequency value, severity value, safe T value,and the effect of productivity. The results knewthat the frequency level was obtained (91,6; 73,6; 53) with an average of 72,7 work accidents that caused injuries for every one million working hours while the severity level was obtained (3328,9 ; 2008,4; 1647,8) with an average of 2328,2 hours lost for every 1,000,000 hours worked, with a safe T-score in 2020 known to be -19 and in 2021 there are -27 if the Safe-T-score is negative that conditions improve. It can be seen that the fewer accidents that occur, the smaller the lost working hours and result in higher productivity levels. Types of potential accidents that occur are eye contact with material debris, inhaling residual air from the lathe process, falling, being pinched by a machine, electrocuting, and falling on objects.Keywords:Implementation of Work Safety Program, Fault Tree Analysis (FTA), Productivity
